How do I enroll? Simply get an enrollment form from your benefits coordinator or from ERS. Or enroll online at www.ers.state.tx.us. You’ll receive your ID card and benefits booklet by mail. Do I have to go to a network provider? No. You may go to any provider. For many services, your costs are lower when you see a dentist in the CONNECTION Dental Network. How do I find a provider in my area? CONNECTION Dental Network offers a large network of participating dentists in Texas and throughout the United States. To find a dentist near you, please go to the Provider Lookup tab, where you can look for a provider by name or specialty. Do I have to have a referral to see a specialist? No. How can you include my dentist in your network? You can nominate your dental provider if he/she does not participate in our network simply by completing a Dentist Nomination Form. Once we receive the nomination form, we will contact your provider. If your provider agrees to participate in the network and qualifies through credentialing, he/she will be added to the network. Do I have to have authorization prior to receiving dental care? No, but it is a good idea. Before you receive treatment, Texas Dental Choice can help you estimate how much your care will cost. Ask your dentist to submit your proposed treatment plan and we will send you and your dentist an explanation of how the services will be covered by your dental plan and outline your costs.
