Dental Choice ID card for a family member, or if you've misplaced your own ID card.
When you register for Member Web Services, you choose your own user ID and password, so only you have access to your account information. Just click on the
Register button above to get started. If you have already registered, click the Log-in button, then enter your member ID number, user ID and password.
